All student-athletes MUST COMPLETE REQUIRED PAPERWORK ANNUALLY to be eligible for practice and/or competition. This includes all medical clearance forms, eligibility forms, and sports information athlete bio forms. If you are a new student interested in trying out for a team, please contact the head coach for additional information & access to the forms.

Read below for a step-by-step guide to completing these forms:

STEP 1: Click on the "MEDICAL CLEARANCE" header and follow the directions for new or returning student-athletes
STEP 2: Click on the "ELIGIBILITY FORMS" header and follow the directions to complete the required NCAA Eligibility Forms
STEP 3: Click on the "SPORTS INFO ATHLETE BIO FORMS" header to fill out additional publicity information for your athlete bio
STEP 4: Review additional resources on the "MORE" header, including Name, Image and Likeness (NIL), Transfer Information, International Student-Athletes, and NCAA Educational Resources
